What Is Secondary Infertility?
Secondary infertility is defined as the inability to conceive or carry a baby to term after previously giving birth.
Secondary infertility affects about 11% of couples in the United States. Of those, about one in five cases are identified as unexplained, meaning doctors cannot identify a clear medical reason for the difficulty in conceiving a second child.
The Difference Between Primary vs Secondary Infertility
Primary infertility occurs when a woman has never achieved pregnancy, even after trying to conceive for 12 months (or six months for those over age 35). Secondary infertility occurs when a woman is unable to conceive again after having at least one successful pregnancy. The causes are often similar, no matter when you experience infertility.
What Are the Causes of Secondary Infertility in Females?
Several factors can contribute to secondary infertility in men and women, including:
- Advanced maternal age (35 and older)
- Issues with sperm production, count, shape, or function
- Damaged or blocked fallopian tubes
- Endometriosis
- Polycystic ovary syndrome (PCOS)
- Pelvic inflammatory disease (PID)
- Other uterine conditions (e.g., fibroids, polyps, scar tissue, radiation damage, injuries)
- Thyroid disorders
- Obesity or drastic weight changes
- Smoking and alcohol use
- Certain medications
What Are Secondary Infertility Symptoms?
Many women do not experience symptoms of secondary infertility.
However, those with underlying medical conditions, like PCOS or thyroid disorders, may experience irregular or absent menstrual periods.
PCOS can cause missed periods due to hormonal imbalances, and thyroid conditions can disrupt regular ovulation.
Testing Options for Secondary Infertility
Diagnostic testing for secondary infertility is similar to those used for primary infertility and may include:
- Pelvic ultrasound to assess the uterus, fallopian tubes, and ovaries.
- Hysterosalpingogram (HSG) to determine whether the fallopian tubes are blocked.
- Blood tests to determine your ovarian reserve and assess hormone levels.
- Semen analysis in the male partner.
