Reciprocal in vitro fertilization (IVF), also called co-IVF or co-maternity, is a unique way for both partners in a lesbian or trans-male partnership to be intimately involved in the biological development of their baby.

How Does Reciprocal IVF Work?
Reciprocal IVF differs from traditional IVF in that one partner is the egg donor while the other is the gestational carrier. With reciprocal IVF, eggs are fertilized with donor sperm to create embryos.
Here’s what you can expect:
Partner A (Egg Donor)
Partner A will undergo ovarian stimulation and egg retrieval.
- Ovarian stimulation: Daily hormone injections for about 8-14 days to help encourage the growth of multiple eggs.
- Egg retrieval: A short medical procedure to collect mature eggs from the ovaries.
Once retrieved, the eggs are manually fertilized with donor sperm in a lab and allowed to develop over three to five days.
Partner B (Gestational Carrier)
Before and during Partner A’s ovarian stimulation and egg retrieval, Partner B prepares their uterus for the anticipated embryo transfer. This involves daily hormone injections and an embryo transfer procedure.
- Uterine preparation: Daily hormone injections for two to four weeks to prepare the uterine lining for embryo transfer.
- Embryo transfer: Partner A’s embryos are transferred into Partner B’s uterus in a procedure similar to a Pap smear.

The timing of reciprocal IVF is crucial. Both partner’s contributions to the process must align perfectly for the best chance of success.
